6歲男病人昨日入院準備大腸鏡檢查，護士於早上約11時為他放置鼻胃喉管，及後聽診及酸鹼測試均未能確認喉管位置是否正確，故於下午展開X光測試，並由一名實習醫生確認X光片，獲確認可以輸注腸道清潔藥物；護士按指示輸藥後約一小時，病人表示感到不適，有腹痛、氣促、血氧飽和度下降的情况，護士即時停止輸藥，再經另一名醫生檢視X光片後，始發現鼻胃喉位置錯誤，隨即為病人抽走肺部內液體。

The incident occurred as the patient with swallowing difficulty was admitted to the hospital yesterday morning (Oct 9) and a nurse arranged an X-ray examination for him to confirm the position of the nasogastric tube in preparation for a colonoscopy.
