調查委員會指，涉事兩名護士中，包括一名負責落藥的護士涉人為疏忽，當時「太緊張藥物濃度」而出現「無意盲點」，忘記要重新開三路活塞；而另一名負責監督的護士則已按照指引做足要求，惟醫院當時未有指引需要檢查輸注強心藥的管道是否暢通，故醫院同樣需要承擔責任。
